These workshops are run by trained professionals and are designed to be interactive. In order for the workshops to run effectively, your camera must be turned ON.
Who is this workshop for?
This workshop is for anyone in the community who wants to get their own care planning in order, and, who may be a support person for a family member or friend with serious illness.
How long is the workshop?
2 sessions (1/week), 1 hour each
*For Webinars: You’ll use the same Zoom link, meeting ID and password (that you receive in your confirmation email) to join each week.
What topics will be covered?
Session 1 – Advance Care Planning: What Matters
How to identify your future Substitute Decision Maker
Why having conversations about what is important to you or a loved one, matters
Session 2 – Participating in Serious Illness Conversations
How to prepare for essential conversations using scripts
Review sample conversations so you can discuss wishes for care when experiencing serious illness
Review approaches to initiating conversations
See different conversation styles
Q&A to help you get started with your own care planning
