Palliative Care

Family Health Team doctors and other clinicians support patients who have life limiting conditions with a knowledgeable and caring approach. We work closely with home and community care services, hospitals and hospice to help people get the physical, emotional and social support needed.

Guelph Family Health Team has led the implementation of a comprehensive palliative care in Guelph by collaborating with other Guelph agencies. This service ensures people can receive the care they need, when and where they need it.
