Medical Assistance In Dying (MAID)

The Guelph Family Health Team is taking a leadership role in supporting our community’s understanding of and approach to providing MAID for interested Guelph residents.

Patients are encouraged to contact their primary care providers (doctor or nurse practitioner) for information. For general information please contact the Guelph Family Health Team:

The Ministry of Health and Long Term Care has established a Care Coordination Service (CCS) with a toll-free referral support line for patients, caregivers and families who have questions about medical assistance in dying. This service is also to help Ontario clinicians to arrange for assessment referrals and consultations for patients requesting MAID.
