Role:
The Guelph Family Health Team is looking for a part-time (0.4 FTE) Nurse Practitioner for up to 14 month contract.
Role Description:
The Nurse Practitioner will function in collaboration with members of the Guelph Family Health Team, Physicians patients, families, and external partners. The NP successfully functions in an expanded role demonstrating expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients, involving collecting and interpreting data, determining a medical diagnosis, and treating complex responses of individuals and families to actual or potential problems. The NP performs quality improvement and administrative activities and participates in policy and procedure development. Other key components of the role include performing research activities; delivering, coordinating, and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and group within the professional community.
Qualifications:
- A minimum of 2 years nursing practice experience as a Nurse Practitioner, with experience in all age groups is required
- Current Registration with the College of Nurses of Ontario in the Extended Class
- Completion of a master’s degree in nursing is an asset
- Ability to function in a full scope NP role in a collaborative and interdisciplinary primary care setting
- Strong health assessment and diagnosis skills
- Self-directed individual with strong team skills
- High degree of flexibility
