Who is this clinic for?

Through referrals from Family Doctors and/or Nurse Practitioners, Guelph FHT psychiatrists consultations for individuals with mild to moderate mental health illness.


What does this clinic look like?

Patients can expect to meet for 90 to 120 minutes for an initial assessment appointment. Any follow-up appointments are 30 minutes in length. This is a short term service supporting Family Doctors or Nurse Practitioners to maintain ongoing responsibility for mental health care.


How do I book an appointment?

All referrals to this service must be completed by a Family Doctor and/or Nurse Practitioner. The Psychiatry clinic does not accept self referral.


Where is it located?

The Psychiatry Clinic is based out of the Guelph FHT head office in Suite 501 at 649 Scottsdale Drive, Guelph. During COVID-19, most of our appointments are being held remotely by way of telephone or secure video link.
