Who is this workshop for?

This workshop is for anyone in the community who wants to get their own care planning in order, and, who may be a support person for a family member or friend with serious illness.


How long is the workshop?

2 sessions (1/week), 1 hour each
*For Webinars: You’ll use the same Zoom link, meeting ID and password (that you receive in your confirmation email) to join each week.


What topics will be covered?

Session 1 – Advance Care Planning: What Matters

  • How to identify your future Substitute Decision Maker
  • Why having conversations about what is important to you or a loved one, matters

Session 2 – Participating in Serious Illness Conversations

  • How to prepare for essential conversations using scripts
  • Review sample conversations so you can discuss wishes for care when experiencing serious illness
  • Review approaches to initiating conversations
  • See different conversation styles
  • Q&A to help you get started with your own care planning

  • Confidence to prompt or continue essential conversations about what matters
  • Information to share with your health care team so they can best support you or your family member when health and wellbeing declines
