Who is this service for?

The Guelph Family Health Team (FHT) Dietitian Services provides care for patients who are interested in improving their nutrition, relationship with food, managing health condition through diet, and/or making healthy lifestyle changes. Patients must live in the City of Guelph and/or have a Family Doctor or Nurse Practitioner within the Guelph FHT to access this service.

What is involved?

Appointments are one-on-one sessions (30-60mins) with a Registered Dietitian. Together, a review is done to determine current eating habits, health goals, and any relevant medical conditions. The Dietitians will then provide collaborative, goal-focused conversations, and individualized nutrition support.

What topics and health conditions does this service cover?

Dietitian support is available for a wide range of topics and health conditions, including (but not limited to):

• Stomach or bowel problems (i.e., Irritable bowel syndrome, Acid reflux, Crohn’s, Colitis, Celiac disease, and Diverticular disease)
• Vitamin and mineral deficiencies
• Changes in appetite or unintentional weight loss/malnutrition
• Bone health/Osteoporosis
• Prediabetes
• Heart health (high cholesterol, high blood pressure)
• Fatty liver
• Pregnancy and prenatal nutrition
• Infant and child nutrition (i.e., starting solids, picky eating concerns, etc.)
• Polycystic ovary syndrome (PCOS)
• Disordered eating or food and mood concerns
• Food allergies and intolerances
• Vegetarian and vegan diets

How do I book an appointment?

Patients are referred to Guelph FHT Dietitian Services by their Family Doctor, Nurse Practitioner, or another member of their care team. Self-referrals are also accepted, please call 519-837-4444 ext. 5203.

What to bring to an appointment:

For the first appointment, please bring a list of any current medications and supplements, health goals, questions, topics to discuss, and a 3-day food journal or log. A food log should include:

• The time of day you ate and drank
• Descriptions of what you ate
• How much you ate (approximately)

Is this service free?

All appointments with your Guelph FHT Registered Dietitian are free as they are OHIP covered.
