Diabetes Care Guelph

Enhancing Access. Improving Care. Supporting Patients.

Diabetes Care Guelph is dedicated to helping people who are living with or who are at risk for diabetes, to have a healthy and fulfilling life. We work closely with local physicians and other health services in our community to ensure people living with diabetes have access to the information and resources they need to manage their diabetes.

Who is this clinic for?

Diabetes Care Guelph seeks to provide care for adults who have been diagnosed with diabetes or pre-diabetes AND either lives, works or has a Family Doctor or Nurse Practitioner in Guelph. We also support patients using insulin pumps.

Diabetes Care Guelph accepts self-referrals. A referral by your Health Care Provider is not required.

What is involved?

Your appointments will be a combination of individual sessions with a Registered Nurse and/or a Registered Dietitian as well as group education workshops. The frequency of appointments is determined on an individual basis and can range from weekly to once a year.

INDIVIDUAL APPOINTMENTS:
During your individual appointments, you will be asked health history questions. We will talk about your medications and your blood work. We will answer any questions that you have and work to set some personal health goals. You may also choose to see our Registered Kinesiologist and/or Mental Health Counsellor.

ONLINE PEER SUPPORT GROUPS:
Our support groups are currently postponed indefinitely. Please check back here or speak to your Health Care Provider if you are interested in this opportunity.

GROUP EDUCATION WORKSHOPS*:
The group workshops offer you the opportunity to ask questions related to diabetes in an informal way. You will have the benefit of hearing the information from our Registered Nurse and Registered Dietitian, as well as support from other people who share a diagnosis of diabetes.

GYM FACILITY:
If you are seeing our Registered Kinesiologist, you will have access to our gym facility, which can be used at no cost to you. You may choose to use this on a drop-in basis only, or you can also participate in group fitness classes.

Do I have to bring anything to my appointment?

For the first appointment, please bring your medications in their original containers. After that, please bring your blood sugar meters and/or log books to all other appointments.

*If you are not feeling well and need to stay home, view the Sick Day guidelines HERE! Call 519-840-1964 (Monday – Friday, 9am to 5pm) if you have any questions or concerns.
