We welcome the following staff to the Guelph Family Health Team Sylvia Scott, Manager Chronic Disease; Helen Beimers-Templeton, MH Counselor; Michaela Devries-Aboud, Health Promoter; Philana Cassolato, Candice Duguay and Pauline Reinhard, RNs with Chronic Disease Prevention & Management

The Guelph Family Health Team is involved in an exciting program with the Ontario Telemedicine Network. The Program represents Phase one of the roll-out of the Ontario Telehomecare Strategy. The program is targeted at the management of two specific chronic diseases, Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD. To date 7 patients are part of the program and more are expected to be added shortly.

After a long period of planning involving the MOHLTC, the Guelph Community Health Centre and the Guelph Family Health Team, the adult diabetic outpatient education service (Type 1 and Type 2 Diabetics) for the Guelph community has been relocated from the Guelph General Hospital to the community effective August 5, 2008. The GFHT will provide the leadership around diabetes education and mangement for Guelph adults, and will support all referring Guelph physicians in their work with diabetic patients, including family physicians outside of the Family Health Team, and specialist support.

Please join us on August 18 2010 for our 2nd annual Diabetes Care Guelph BBQ. This event will be from 5-7 pm and is open to all FHT staff, their families, patients and community members. The event will take place at Riverside Park again (under the red roofed shelter). We will have food (small charge), entertainment and stations set up with diabetes information from several disciplines.
